We want to make it as easy as possible for you to ask for a copy of your medical records. Release of Information for all UChicago Medicine Health System locations is now centralized. This means that records from any UChicago Medicine facility can be requested together and separate requests are no longer needed.

Who can sign to get health information for a patient:

  • The patient (Not the spouse.)
  • Power of Attorney of Healthcare if the patient is not able to sign. (Legal document must be provided.)
  • Parent (If the patient is younger than age 18 years of age.)
  • Parent and minor if the patient is 12 to 17 years of age and is getting psychiatric, alcohol, or drug treatment services.
  • Legal guardian (Proof of guardianship document must be provided.)
  • Representative of the estate for patients who have died. (Copy of the death certificate and a copy of the representative of estate documents must be provided).

If your relative has died, you can ask for copies of their medical records if:

  1. You are the power of attorney for health care that goes beyond the patient’s death or you are the administrator of estate. You must give a copy of the legal document and the death certificate.
  2. You are an authorized relative if you are the surviving spouse of the patient who has died. If there is no surviving spouse, you are an authorized relative if you are the eldest child, a parent, sibling or other relation that is named and defined in the Illinois rules of intestate succession (755 ILCS 5/2-1).

How to request the records of a relative who has died:

  1. Download an authorization form to allow UChicago Medical Center to release the medical records of your deceased relative.
  2. Download and fill out the Authorized Relative Certification
  3. Send the authorization, a copy of the administrator of estate, the relative certification and a copy of the death certificate to the location of the patient’s visit as noted above.

Please allow 7 to 14 business days for processing.

Cost of Records

  • There is no cost to patients asking for their own medical records.
  • There is no cost for records going directly to other healthcare providers.
  • Requests for third parties are charged the State of Illinois copying fees (735 ILCS). Records are not released until we receive payment.
