If you would like to send testing to UChicago Medical Laboratories, you will first need to set up a one-time client account.  

How to Ship Your Samples

Ensure you have a Requisition filled out with your Client Account Code. (If you need a requisition, please contact us via email: uchicago.medlabs@uchospitals.edu.

Before you send your specimen, please be sure to follow the proper procedures for preparing, collecting, packaging and shipping to our facilities. This will ensure you receive the most accurate test results possible. To make this easier, we have created specimen-handling protocols to assist you in collecting and shipping appropriate samples.
